GMUHS Health Services This is the annual reminder to parents of new 7th graders for the Fall of 2010; if your student is planning on participating in GreenMountain sports program, a current well child physical must be on record at the high school. It must be within the last 2 years to be considered current. Forms for the PE can be picked up at the main office of the high school or from the school nurse. Please schedule your appointment with your MD soon, because physicals are time consuming and the MD schedules well in advance.
